Concentric to Epic eForms transition survey

University Hospitals Plymouth (UHP) is transitioning from using Concentric for digital consent to managing consent via eForms within Epic, as part of the wider Epic roll-out in July 2026.

As the first organisation to move away from Concentric, both the Trust and Concentric Health teams are keen to understand what this change means for clinicians in practice. Responses will influence how the Trust approaches and supports the upcoming transition.

The survey has been developed by me, Orla Mantle. I'm a final-year medical student at King’s College London, currently completing my medical elective with Concentric Health.
The survey takes approximately 5 minutes to complete. It asks about your experience using Concentric and your expectations regarding the transition to Epic eForms. Responses are confidential and reported only in aggregate.
A second survey will open for responses after the transition to assess comparative experiences and reflections on the transition. Those who complete both the initial and follow-up surveys will be entered into a draw to win 1 of 3 £50 Amazon vouchers.
